iTOCex: Interprofessional Transition of Care Experience
What is iTOCex?
iTOCex is an innovate interprofessional learning opportunity for health professional students. The unique student experience follows a patient’s care from discharge at the hospital to an outpatient pharmacy visit to a follow-up appointment with their provider. While we are actively working to expand the simulation to include more health professional programs, currently the simulation is intended for pharmacy, nursing, and prescriber (MD/NP/PA) students.
The experience highlights breakdowns that occur at a systems-level during transitions of care and the subsequent impact on patients’ health outcomes. Students have the unique opportunity to play both the practitioners providing direct care to the patient and system leaders performing a root-cause analysis after the system breakdown.
One of the main challenges to interprofessional education is finding other health professional programs to participate with. Our innovative interprofessional experience alleviates this barrier by allowing health professional programs from separate schools to participate in an interprofessional learning opportunity together!
